Dr. Tibb is a front line clinician, researcher and educator. He joined Valley Health System in July 2014 and currently serves as Chief Quality Officer of Valley Medical Group in addition to being the Medical Director of Palliative Care at The Valley Hospital. Dr. Tibb is Board certified in Internal Medicine, Pulmonary Diseases, Critical Care, Neurocritical Care and Palliative & Hospice Medicine.  Dr. Tibb also completed a fellowship in Clinical Quality and Patient Safety and his work has been recognized both locally and nationally with several awards for Quality Improvement.  In 2012-13 and 2013-14 he received the 'GAGE' award from America's essential hospitals for "Palliative Care Integration in the ICU" & "CAUTI reduction – Journey towards ZERO" respectively. 
At Valley Medical Group, Dr. Tibb is involved with Population Health and is instrumental in using Quality Metrics to drive physician compensation.  He was formerly the Medical Director of the Intensive Care Unit at Jacobi Medical Center in New York City where he also served as Attending Physician on the Pulmonary, Critical Care and Palliative Care services.  He still retains faculty position as Clinical Assistant Professor of Medicine at Albert Einstein College of Medicine.  In addition, Dr. Tibb completed a fellowship in Medical Simulation and held faculty position at the Institute of Medical Simulation and Learning (IMSAL).  Dr. Tibb has served as faculty for numerous medical societies including several Critical Care Ultrasound courses over the last 7 years and has given grand rounds on integration of Ultrasound in the Intensive Care Unit both nationally and internationally. He has several peer reviewed publications and his exemplary work as an educator has been recognized in the form of "Best Educator" award for 3 consecutive years from 2011-2014.  He is a standing Fellow of the American College of Chest Physicians.  He lives in New York City with his wife and 3 children.
1. List and evaluate evidence-based interventions shown to reduce heart failure readmissions
2. Describe effective methods of providing education to heart failure patients prior to discharge
3. List and describe the factors that may influence the rate of heart failure readmissions.
4. Assess practice-based clinical metrics to determine factors contributing to high or low performance.
5. Recognize the leadership and interpersonal skills necessary for successful Quality Improvement initiatives
6. Apply guideline-directed medical therapy for patients with heart failure
